1. Fill out the following form to the best of your ability. Please make sure every question is answered as detailed as you require the tattoo to be.

2. Allow me time to respond to you via email. I do not respond to DMs or any other form of communication.

3. Please only send in 1 request, emails will be responded to in the order they come to the inbox. Sending multiple requests will place you further in the line as your request will be brought to the top of my inbox, and not the bottom.

4. The design will be sent to you anywhere between 1 week to 2 days prior to your tattoo appointment. Once you have approved it, only minor details can be changed on the day of such as sizing and placement. The nature of my tattoo style is very painting heavy, therefore anything that requires me to do any more painting on the day of will have to be rescheduled. Furthermore, I want you to love this tattoo, help me do this by giving me clear communication and direct input from the very beginning!

5. Please note, if you fail to give approval to your design, fail to show up, or reschedule/cancel within 2 days before your scheduled appointment, you will lose your spot in the 100 free tattoos, and will have to wait until the 100 is over in order to book again.

Tipping Policy:

As stated in the advertising, this tattoo is free. However, tipping is always appreciated and welcome, as I am not paid to work full time in the tattoo shop.

Any tips I am given will be used towards shop supplies so that I can keep doing the 100. Thank you in advance!
